This policy is for residents of Washington and Nevada. It explains how Arise Treatment Homes LLC dba Pinnacle Health Group handles consumer health data — information that identifies you and relates to your past, present, or future physical or mental health.
Most health information is not covered by this policy
We are a HIPAA covered entity. Information we hold as your treatment provider is protected health information, governed by HIPAA, the federal substance use disorder confidentiality rules at 42 CFR Part 2, and California’s Confidentiality of Medical Information Act, and described in our Notice of Privacy Practices. Washington and Nevada law exempt that information from this policy.
This policy covers health-related data we collect through our website that falls outside HIPAA — for example, what your visit to a page about a particular condition or treatment may suggest, before you ever contact us.
What we collect
- Browsing activity. The pages you view on this site can indicate an interest in a particular condition or treatment. We collect page views and referral information.
- Identifiers and device data. IP address, device and browser type, and cookie or device identifiers. This is not stored on our web server.
- What you submit. Contact and insurance verification forms ask for your name, contact details, insurance information, and what you tell us about your situation. Once you contact us about treatment, we handle that information as protected health information under HIPAA.
Where it comes from
Directly from you, automatically from your device when you use the site, and from advertising and referral partners who tell us which ad or listing sent you here.
Why we collect it
To answer your inquiry, to check insurance benefits when you ask, to operate and secure the site, and to measure website and marketing performance.
Who we share it with
We may share consumer health data with these third parties:
- CallTrackingMetrics — call tracking and form handling
- Dazos — our customer relationship system
- Kipu Health — our electronic health record system
We may also share it with the affiliated treatment programs operated by Arise Treatment Homes LLC, so we can route your inquiry to the right program.
Each of these providers has a business associate agreement with us and may use the information only to do work for us.
We do not sell consumer health data. Washington and Nevada define “sale” broadly. If that ever changes, we will get your signed authorization first, separately from any other consent.
How long we keep it
We keep consumer health data only as long as we need it for the purpose we collected it, or as long as the law requires.
Your rights
If you live in Washington or Nevada, you may:
- Confirm whether we collect, share, or sell your consumer health data
- Get a list of the third parties and affiliates we shared it with, with contact information for each
- Withdraw your consent to our collection and sharing of it
- Delete your consumer health data
We will confirm we received your request and respond within 45 days. If we need more time, we will tell you within those 45 days and may take up to 45 more.
If we deny your request, we will explain why, and you may appeal by replying to our decision. We will respond to an appeal within 45 days. If we deny the appeal, we will give you a link to submit a complaint to the Washington Attorney General or the Nevada Attorney General.
We will not deny you care or charge you differently because you exercised these rights.

We will ask for enough information to verify your identity and to confirm you live in a state covered by this policy. You may use an authorized agent; we may ask for written proof of their authority.

We will post changes here and update the date above.
