Pinnacle Health Group is a brand of Arise Treatment Homes LLC (“we,” “us”). This policy explains how we handle information on pinnaclehealthgroup.org.

What this policy covers

We are a HIPAA covered entity. When you contact us about treatment — through a form, a call, or an email — the information you give us is protected health information. That information is governed by HIPAA, the federal substance use disorder confidentiality rules at 42 CFR Part 2, California’s Confidentiality of Medical Information Act, and our Notice of Privacy Practices. This policy does not authorize any use or disclosure of protected health information.

What we collect

Information you give us. Your name, phone number, email address, mailing address, insurance information, and whatever you choose to tell us about your situation or the person you are contacting us about.

Calls. Calls to and from our numbers may be recorded. When a call is recorded, we tell you at the start of the call and record only with your spoken approval.

Information collected automatically. When you visit the site, we and our providers may receive your IP address, device and browser type, the pages you view, and the site or ad that referred you. We collect this using cookies and similar technologies. It is not stored on our web server.

Information from others. Insurance carriers confirm benefits when you ask us to check coverage. Advertising and referral partners tell us which ad or listing sent you here.

Cookies, analytics, and advertising

We use cookies and similar technologies to operate the site and to measure website and marketing performance.

We do not sell your information. Some state laws treat the use of advertising technology as “sharing” personal information for targeted advertising. You can opt out — see Your choices below.

Where a provider handles information subject to HIPAA, we have a business associate agreement with them.

Who we share it with

Service providers who help us operate the site, handle inquiries, and communicate with you. They may use the information only to do work for us.
Affiliated treatment programs operated by Arise Treatment Homes LLC, so we can route your inquiry to the right program.
Insurance carriers, when you ask us to verify your benefits.
Others when the law requires it, including in response to a subpoena, court order, or a request from a licensing or public health authority. Disclosures of protected health information follow HIPAA and California law, not this paragraph.

How long we keep it

We keep information only as long as we need it for the purpose we collected it, or as long as the law requires. Medical records are kept for the period California law and our licensing requirements set.

Security

We use HTTPS encryption, access controls that limit what staff can see, and written agreements with providers that handle protected health information. No website or database is completely secure, and we cannot guarantee that information sent to us will never be accessed by someone else.

Your choices

Targeted advertising. To opt out, email marketing@pinnaclehealthgroup.org or call (888) 840-3534.

Opt-out preference signals. We honor the Global Privacy Control as a valid request to opt out of the sale and sharing of personal information. It works per browser and per device, so you will need to set it on each one you use.

Calls, texts, and email. Reply STOP to any text to stop texts. Use the unsubscribe link in any marketing email.

Your rights

We extend these rights to every U.S. resident:

These rights apply to website data. Your rights in your medical record — access, amendment, restrictions, an accounting of disclosures — come from HIPAA and are described in our Notice of Privacy Practices. California law exempts medical information governed by HIPAA and the Confidentiality of Medical Information Act from the state’s consumer privacy law.

How to make a request. Email marketing@pinnaclehealthgroup.org, call (888) 840-3534, or write to us at the address below. We will ask for information to verify your identity. We respond within 45 days, and will tell you if we need up to 45 more.

You may use an authorized agent. We may ask the agent for written permission from you.

We will not deny you care, charge you differently, or give you a lower level of service because you exercised a privacy right.

California residents have the rights above under the California Consumer Privacy Act, plus the right to know the categories of personal information we disclosed in the past 12 months. We do not knowingly sell or share the personal information of anyone under 16.

Children

This site is for adults. We treat patients 18 and older and do not direct this site to children. We do not knowingly collect personal information from anyone under 18. If you believe a minor gave us information, email us and we will delete it.
